Introducing the
Care GuidanceSM Program
Helping individuals maintain or improve their health is the goal the Care Guidance Program. Funded by Medicare and developed by Montefiore Medical Center, the program will provide care management services to selected fee-for-service Medicare beneficiaries in the Bronx.
The Care Guidance Program is designed to:
- Promote better health
- Reduce emergency room visits and hospital admissions
- Help prevent medication complications
- Coordinate the healthcare needs of program members
UPDATE:
The Care Guidance Program is pleased to announce that as of March 1, 2007 more than 1,690 Medicare beneficiaries are participating in the program. We would like to thank our participating beneficiaries for taking part in this unique three-year Medicare demonstration/pilot, which began operations on June 1, 2006. Your participation in the Montefiore Care Guidance Program can help us to better understand and address the needs and concerns of Medicare beneficiaries with chronic or recurring health problems.
If you are not currently participating in the Montefiore Care Guidance Program, we would like you to consider if this program is right for you. You can use the services offered through the Montefiore Care Guidance Program as little or as much as you want at no extra cost beyond your regular Medicare premiums, co-payments and deductibles. A Care Manager will work with you and your physicians to manage and coordinate your health care needs over the phone so that you don’t have to leave the comfort of your home. And you can always call your Care Manager if you need any of the services available. We can also provide support to anyone who helps take care of you.
A team of Care Guidance health care professionals including physicians, registered nurses, patient educators, a pharmacist, and a nutritionist are available and ready to provide assistance.
Remember, the Care Guidance Program is NOT an insurance plan, prescription drug plan, HMO, or Medicare Advantage Plan. You remain in original Medicare with all your Medicare benefits. If you enroll in a Medicare Advantage Plan (managed care), you cannot participate in the Montefiore Care Guidance Program. |
